We have now made available the Fling Charters/Spree Expeditions/Gulf Diving, LLC Voluntary Release, Waiver and Assumption of Risk Form for your convenience. In the past the divers where given this form to fill out as they board the boat. Each and every diver/non-diver must complete this form before the boat leaves the dock.

All the blanks must be filled in for the exception of Bunk # and you will do this once you have selected your bunk. Please be sure that you initial all the blanks on page 2.

Unaccompanied minors (under the age of 18) need the following forms signed by parents/legal guardian and notarized:
